These five questions come first because they matter more than the rest of this questionnaire. Please answer honestly.

Have you ever felt physically unsafe with this person, or been physically hurt by them?
Has this person — or someone connected to them — followed, monitored, or contacted you after you asked them to stop?
Right now, do you feel you may be in immediate danger?
